Fraser Case Management – helping you access and navigate complex, social and medical services

Fraser provides the following types of case management services to people of all ages in Dakota, Hennepin, and Ramsey Counties.

  • Brain Injury (BI) Waiver
  • Community Access for Disability Inclusion (CADI) Waiver
  • Developmental Disability (DD) Waiver
  • DD Non-Waiver

You can be referred to Fraser following an assessment by your county of financial responsibility. After a referral, you are matched with a case manager, who will help you develop a service plan, tell you about different resources and services, help you connect to those services, evaluate the effectiveness of services and provide other needed support.

Career Planning and Employment – Helping Individuals with Autism, Mental Health Needs, and Disabilities Find Success in College and Career

Fraser Career Planning and Employment (CP&E) Services supports individuals with neurodiversity, mental health, and intellectual or physical disabilities to prepare for success in college or career. We offer a range of services funded through MN Vocational Rehabilitation, HCBS or CDCS Waivers, employers, or self-payment. Meetings can be virtual or in-person. Behavioral Health Home

With Fraser Behavioral Health Home services, you can focus on your child, while we focus on coordinating care with the right experts.

Fraser’s Behavioral Health Home team has a registered nurse on staff, and the expert team knows what medical and mental health services are available for your child and your family. Behavioral Health Home services are for children who receive Medical Assistance (MA), MA through a Managed Care Program (PMAP) or are on the Tax Equity and Fiscal Responsibility Act (TEFRA).

We support your family by managing your child’s health and wellbeing. There are monthly follow-ups, either in-person or by phone, to share updates on goals and progress. You will also meet with a team member every six months to review and discuss wellness status and your health action plan.

  • Care coordination
  • Health promotion and wellness
  • Comprehensive transitional care
  • Patient and family support
  • Referral to community and social support services

Fraser Home-Based Services

Fraser Home-Based Services offers personalized support to individuals and families, promoting skill development, independence, and confidence. Delivered by trained professionals and supervised by qualified coordinators, these flexible services can be provided on an hourly, daily, or weekend basis. Our services focus on community participation, health and wellness, household management, and adaptive skills.

Service Types:
  • Individualized Home Supports (IHS) with Training: For adults needing support and training in community living categories, residing in their own or family home.
  • IHS with Family Training: For adults or children in their family’s home, providing both support and training to the individual and family.
  • IHS without Training: For adults and children requiring support, assistance, or supervision in community living categories, in their own or family home.
  • Respite Care: Temporary relief for primary caregivers, including support with daily tasks, companionship, meal prep, personal care, and light housekeeping.

These services are tailored to meet each individual’s needs and ensure they thrive in their home environment.
