Behavioral Health Home

With Fraser Behavioral Health Home services, you can focus on your child, while we focus on coordinating care with the right experts.

Fraser’s Behavioral Health Home team has a registered nurse on staff, and the expert team knows what medical and mental health services are available for your child and your family. We support your family by managing your child’s health and wellbeing. There are monthly follow-ups, either in-person or by phone, to share updates on goals and progress. You will also meet with a team member every six months to review and discuss wellness status and your health action plan.
