Break Out Session Information for Resources, Connections and Belonging: Navigating the transition into adulthood for neurodiverse emerging adults

Choices for Break Out Session A - 11:20 AM to 11:10 AM

Option 1:  Self-Advocacy Strategies: Fraser Self Advocate Advisory Council (SAAC)

A panel of Self-Advocates will share their lived experience and offer strategies to encourage self-advocacy.

Option 2: IEP/504 Transition Planning: Sue Fager, Transition to Adulthood Coordinator, PACER

Join us for an insightful session where we explore the transition planning process, providing you with the tools to prepare for life after high school. Understand the fundamentals of the special education process, discover key transition services, and learn how to effectively collaborate with the school team to ensure a smooth transition into adulthood.

Option 3: Executive Functioning Strategies: Katie Norris, MA, LPCC, Fraser OnTRAC Service Lead

This Break out session will provide very brief definitions, common challenges and strategies to support Executive Functioning.  Everyone has to learn and practice executive functioning skills– the benefit of this is to maximize efficiency to do the things we ‘need’ to do so that we have the time/space/energy to do what we want/what’s important to us!

Option 4: Accessing Social Security Benefits: Tanya D. Heitzinger, Medical Relations Supervisor, Mn DEED and Sara Cavallo, Area 1 Work Incentives Coordinator, Mn DEED

The Minnesota Disability Determination Services is the state agency that partners with the Social Security Administration to make the medical decision if an individual qualifies for the Social Security Disability program. This presentation will provide you resources on applying for benefits, submitting a complete application, working while on benefits, and a whole lot more.

Option 5: Parent Support: The Moreland Family

The Moreland family will share their story in the hope that others may find hope and to help create a more accepting, inclusive world. In our presentations, we offer the “whole family” perspective on living with autism and include struggles, insights and lessons learned


Choices for Break Out Session B - 11:30 AM to 12:20 PM

Option 1:  Neurodiversity Through the Lifespan: Tony Geckler, MSW, LGSW

Tony will share his life experience through a neurodiverse lens and how individuals can use it in their practice.

Option 2: Accessing Post-Secondary Supports: Julie McKibbins, Fraser Career Planning and Employment;  Kristin Burgess, Metro State University; Melissa Fletcher, Dakota County Technical College; and Allison Jenson, Inver Hills Community College

Getting ready for college, trade school, or university? You don’t have to navigate it alone. This session will introduce you to the wide range of resources, services, and supports available to help you succeed after high school — from academic accommodations and financial aid to mental health services, tutoring, and more. Learn how to advocate for yourself, access what you need, and thrive in your post-secondary journey!

Option 3: Accessing Employment Supports: E1MN partners

Join us for a focused session on preparing individuals for the workforce. Learn about practical strategies and resources for developing job readiness skills, navigating the employment landscape, and connecting with career opportunities that align with individual strengths and interests.

Option 4: Accessing Case Management and Waivers: Sarah Shafer, LISW  Senior Director, Fraser Case Management

Do you want more information about the steps to take to access and fund services to help you learn how to navigate adult responsibilities? If so, then this is the breakout session for you! I will present information about the four disability waivers, waiver and non-waiver case management, and a bit about what Fraser offers in this realm. 

Option 5: Navigating Metro Transit: Doug Cook, Metro Transit Trainer and Joseph Blustein, Employment Development Specialist, Fraser Career Planning and Employment

Learn how to navigate Metro Transit buses and rails. You will have an opportunity to board and ride a bus simulator, get tips on how to ride and problem-solve.  *This session will be held outside on the simulator bus in an adjacent surface parking lot.


Choices for Break Out Session C - 02:00 PM to 02:50 PM

Option 1:  Building Connections Activity:  Speed Friending   Julie McKibbins, Assistant Program Manager, Fraser Career Planning and Employment 

 Be Brave * Follow our Lead * Share your Authentic Self * Make New Connections

Speed Friending is a wildly fun activity to meet new people and make social connections. We took the awkward small talk out by providing you with interesting conversation cards that are thought provoking and conversation-worthy.  Join us for fun and laughter.

Option 2: Metro State University Tour

Discover Your Future – Join Our Campus Tour! Ready to picture yourself as part of our vibrant college community? Come explore our beautiful campus and get an inside look at academic life and student activities.

Option 3: Physical Health and Wellbeing:  Nathalie Lechault, MD  Lead Clinician for Transition HealthCare Initiative South Lake Pediatrics  and Tara Mahin Care Coordination Supervisor, South Lake Pediatrics

Join South Lake Pediatrics for an informative session on effectively managing the shift from pediatric to adult healthcare. Learn key strategies and resources to ensure a smooth transition and continuity of care for young adults.

Option 4: Decision Making Supports:  Kathleen Carlson, LSW,  Volunteers of America

There are many options to support young adults with decision making whether it’s related to finances, medical decisions, housing, education, and more! Join us to explore a spectrum of decision-making options from the least restrictive approaches through guardianship.

  Option 5: Parenting Neurodivergent Humans: Dr. Morgan Beidleman

Parenting a neurodivergent human presents unique challenges and powerful opportunities for growth, connection, and advocacy. Join our keynote speaker for an insightful conversation about the realities of parenting neurodivergent individuals and discover ways to navigate this journey with empathy and strength.


Choices for Break Out Session D - 03:10 PM to 04:00 PM

Option 1:  Building Connections Activity:   Joseph Blustein, Employment Development Specialist, Fraser Career Planning and Employment

Get creative and connect! Join us for a hands-on, interactive activity designed to spark creativity and connection as we deconstruct laptops to uncover its inner workings — then flip the script to build your very own “can-do” robot using upcycled computer parts and shop materials. This unique experience blends tech exploration, tool skills, and creative reuse – perfect for curious minds, future makers, and anyone looking to build connections while building something awesome.

Option 2: Living your Best Life using Person-centered practices:  Amy Pagett, Onboarding and QA Specialist, Fraser Case Management

Join us for an interactive workshop designed to empower you with person-centered practices. Learn what person-centered thinking is, discover what truly matters to you, and gain practical skills to advocate for yourself. Explore the importance of your support network and harness the power of 'I' to take control, dream big, and choose your best life.

Option 3: Money Management Strategies: Old National Bank

Managing money can feel overwhelming-but building financial confidence starts with a few simple strategies.  Whether you’re earning your first paycheck or planning for future goals, this session will help you build habits that support independence. 

Option 4: Estate Planning and Long-term Financial Supports: Joseph P. Kukla, esq. Thiel, Anderson, Kukla & Gunderson, PLLP

Explore strategies for estate planning and providing long-term financial support for family members with disabilities. Attendees will gain insights into creating a secure and sustainable future for your loved ones.

Option 5: Accessing Housing Supports: PACER  

Finding the right place to live is a big step- and you don’t have to do it alone.  In this session, we’ll explore different types of housing options available and the various supports that can help you along the way.  Whether you’re just starting to think about living on your own or are ready to take the next step, this session will provide helpful information, resources, and guidance to support your housing journey.
