We feel it's important to let you know that Fraser has a waiting list for most services.

Speaking with our intake coordinators

When you call, you should expect to be on the phone for 10-15 minutes and ready to provide the following information:

Your contact information
Insurance information
Brief medical history and diagnosis, if any
Referred to Fraser by whom? (name, address, phone)
Specific Fraser program you are calling about or have been referred to
Current treatment, if any (dates and providers)
Previous evaluations and testing, if any (dates and providers)

Next steps

Once your intake phone call is complete, you will receive a registration packet that captures the information you provided over the phone and asks for additional information (if necessary), plus your signature. If you are placed on a waiting list, the intake coordinator will contact you as soon as an appointment becomes available.
