Home-Based Services

Fraser® Home & Community Supports

The Fraser Home-Based Services program offers a menu of service choices, many of which can be combined to best meet the needs of each person receiving services and his/her family.  Services are supervised by qualified coordinators and implemented by trained direct support professionals.

This program is offered through the Fraser Home & Community Supports division which assists people with maintaining physical and behavioral health, family well-being, increasing independent living skills and participating in the community through personal care assistance, contracted and flexible case management, respite care, personal and in-home family support.

Menu of Services Provided

In-Home Family Support is a licensed service provided in a person’s home and in the community directed toward increasing and maintaining the person’s physical, intellectual, emotional and social functioning. Habilitation is the focus of this program and may be provided not only to the person receiving services but also training the family members to increase their capabilities to care for and maintain the person in their home. Individual program and behavior support plans are developed based on the service plan and following a thorough assessment of the person’s strengths and needs. Targeted goals are developed and measured based on a collaborative team process.

Respite Care is offered in the person’s home to provide short-term relief for the primary caregiver of an individual or individuals with special needs. The intent of the program is to increase family well-being, to prevent maltreatment and out of home placement. Services are provided on an hourly, daily or weekend schedule, depending on need and service authorization.

Personal Support services provide supervision and assistance in the person’s home or in the community to achieve increased independence, productivity and inclusion in the community in accordance with outcomes identified in the service plan, but when skill training is determined not to be necessary to attain these outcomes. Personal Support services are non-medical and are not considered habilitative.

Personal Care Assistant (PCA) services provide assistance and support to eligible persons with disabilities and special health care needs who live in the community. Services are provided by a qualified personal care assistant (PCA) and may be provided in the person’s own home or in the community where a person may go in a typical day and included on the PCA Care Plan. Services are designed by maintain functioning and are not considered habilitative as formal training is not involved.

Other Services Fraser is also contracted to provide additional home-based services depending upon funding source and county such as 24-hour Emergency Assistance, Assistive Technology, Chore Services, Environmental Modifications, Extended PCA Services, Homemaker Services, Specialist Services and Transportation.
