Contracted Case Management

Fraser® Home & Community Supports

Fraser Contracted Case Management services are designed to help individuals navigate the complex social service system.

This program is offered through the Fraser Home & Community Supports division which assists people with maintaining physical and behavioral health, family well-being, increasing independent living skills and participating in the community through personal care assistance, contracted and flexible case management, respite care, personal and in-home family support.

Services provided include a comprehensive assessment of an individual’s healthcare, social, housing, educational, vocational and other services needs. The service planning process is person-centered and includes informing the individual of the waiver, state-plan and other service options in reference to his/her assessed needs and preferences. Once an individual is fully informed of the choices, an individual service plan is developed. Individualized support and advocacy are provided to assist the individual to access the services identified in the plan. Coordination, monitoring and evaluation of those services are also provided to assure health, safety and quality service delivery.

Individuals of all ages referred by Hennepin County for contracted case management.

Our Staff

Fraser contracted case management staff are committed to excellence and meet or exceed the professional qualifications required by the case management Minnesota Rule 185.

Location

Contracted case management services are provided at the client's home, school, worksite or other client preferred community location. Fraser Home & Community supports is located at: Fraser at Hennepin Square, 2021 East Hennepin Ave, Suite 402, Minneapolis, MN 55413.

Payment/Funding

The following types of waivers are accepted:
  • Developmental Disabilities (DD - formerly MR/RC)
  • Community Alternatives Disabled Individuals (CADI)
  • Traumatic Brain Injury (TBI)
