Fraser® Home & Community Supports assists people with maintaining physical and behavioral health, family well-being, increasing independent living skills and participating in the community personal care assistance, contracted and flexible case management, respite care, personal support and in-home family support.

Ages Served
Children and adults who have developmental, cognitive or other disabilities.
Our Staff

- State certified flexible case managers
- Staff trained in client and family-centered approaches
- Culturally competent
- Spanish speaking staff available
- Behavior intervention training provided for staff
Location
Services are delivered in the person's home and during community activities.
Payment/Funding
Waiver funding, Medical Assistance/TEFRA (for PCA services), Hennepin County Human Services Dept., and private pay.
