Waivered Services

Fraser Community Supports

The Waivered Services Program at Fraser is designed to prevent or postpone the need for people with disabilities to move from their homes. This program is through Fraser Community Supports, which provides people with the assistance that they and their families need to maintain and improve personal health, increase independent living skills and assure community participation. Fraser staff members assess needs and work with families to design custom-made programs. Services are tailored based on each person's needs relating to daily living, health, community involvement and behavior.

Services Provided

Respite Care is short-term relief for the primary caregiver of an individual who has a disability. The type, amount, and duration of service are predetermined, approved by the county, and implemented as part of the client’s Individual Service Plan. Services are provided on an hourly, daily, or weekend schedule, depending on need.

In-Home Support services include program development, implementation, coordination, supervision, assistance, and training or monitoring in the home or community. Fraser partners with the client and family to develop a tailored direct support program plan which includes a monthly meeting with the coordinator, staff, and family.

Personal Support services include non-medical care, supervision, and assistance provided in the home or community with the focus of increasing independence, productivity, and inclusion in the community.

Flexible Case Management services include an individualized assessment of client and family needs; identification of resources; facilitation of communication with other agencies; and assistance in gaining access to services and support. Help may also be provided to develop health and safety plans and to identify future goals and indicators ensuring the plan’s effectiveness. Ongoing assistance may be provided to revise support plans so that they continue to meet client needs and remain directed toward outcomes. Support is also provided to families managing their own waivers under the Consumer Directed Community Supports (CDCS) option.

Children and adults of all ages who have developmental, cognitive or other disabilities.
