Case Management Services help families to obtain and coordinate educational, health, mental-health, recreational, social, vocational, and related services. Case Managers will assist eligible individuals in gaining access to needed medical, social, educational, and other services to address their mental health needs.
These services are available through the Fraser Autism Program and the Child & Family Mental Health Program. Eligibility for case management services includes meeting county criteria for Severe Emotional Disturbance and requires formal authorization through the county.
Children and adolescents, ages birth to 18 who reside in Hennepin or Carver County.
Case managers are mental health practitioners with bachelor degrees and 2,000 hours of supervised experience.
