Fraser Home & Community Supports
In-home services for children, adolescents and adults with special needs
Fraser Home & Community Supports assists people with maintaining physical and behavioral health, family well-being, increasing independent living skills and participating in the community personal care assistance, contracted and flexible case management, respite care, personal support and in-home family support. Fraser staff members assess needs and work with families to design custom-made programs. Services are tailored based on each person's needs relating to daily living, health, community involvement and behavior.
Services include personal care assistant (PCA), respite care, in-home family support, personal support, flexible case management (CDCS) and contracted case management.
The goal of these high-quality, specialized services is to prevent or postpone the need for people with disabilities to move from their homes. Fraser in-home staff are well trained, dedicated and caring.
