Work Schedule: Monday through Friday, 40 hours per week
Job Duties:
- To provide home based and/or center-based case management services to children diagnosed with Severe E/BD.
- Coordinate intakes and discharges to case management services in adherence with policies and procedures.
- Communicate and coordinate with other agency personnel regarding family or child services, including county, public school, public health, etc.
- Modify services to comply with changes in case management regulations.
- Provide information and resources regarding the child and family’s mental health, physical health, vocational, social, housing, educational, recreational, and other service needs.
- Collaboratively develop a thorough Individual Family Community Support Plan (IFCSP).
- Services will be provided in a culturally responsive manner recognizing the unique and multiple cultural, medical, psychiatric, and physical needs of each individual.
- Provide consultation and communication with Child and Family Mental Health (CFMH) program staff.
- Complete documentation in a timely manner as outlined in the regulations and in accordance with Fraser’s policies and procedures.
- Accurately document services delivered and submit all billing to Accounting Department within monthly requirements.
Qualifications Required:
- BA/BS in one of the behavioral sciences or a related field including, but not limited to, social work, psychology, or nursing from an accredited college or university.
- Must have completed 2000 hours of supervised experience in the delivery of mental health services to children, while also receiving regular ongoing supervision totaling 38 hours per year.
- Ability to perform office related duties as well as carry out responsibilities in home and community based settings. Ability to work off-site and travel as needed.
- Ability to achieve and maintain a status of “not disqualified” upon completion of criminal background studies.
- Possession of a valid Minnesota Driver’s License and maintenance of a satisfactory driving record consistent with Fraser policy.
Qualifications Preferred:
- Masters in social work or related field is preferred.
