Work Schedule: Monday through Friday, 40 hours per week, with some flexibility
Job Duties:
- Coordinate intakes and discharges in adherence with policies and procedures.
- Remain abreast of and follow all waiver regulations in the delivery of in-home waivered services.
- Communicate staffing needs to Human Resources as outlined in the Home & Community Supports staffing procedures.
- Monitor utilization of staff hours to avoid over or under utilization.
- Coordinate services with other professionals, such as physicians, mental health professionals, and special education teams, as applicable.
- Provide culturally responsive services to individuals served.
- Provide job training, direction, and supervision as required by regulations, Fraser policies and procedures or as needed to Mentors.
- Provide leadership and guidance to Mentors, including consultation for problem solving and conflict resolution.
- Promote professional growth and job satisfaction of staff by motivating, developing, and coaching.
Qualifications:
- Education and/or Experience: BA/BS in Human Services Field with the equivalent of one year of full-time work experience with an individual/s with a developmental disability required. One year of supervisory experience preferred.
- Certificates, Licenses, Registrations: Meet qualifications of Designated Coordinator.
- Language Skills: Excellent verbal and written communication skills using the English and Spanish language, attention to detail and organizational skills.
- Mathematical Skills: Ability to perform basic mathematical skills, interpret accounting reports, and provide accurate billing information.
- Physical Demands: Ability to demonstrate physical care skills as outlined in the individualized service plans.
- Work Environment: Ability to perform office related duties as well as carry out responsibilities in home and community based settings.
- Reasoning: Ability to understand and implement all related regulations and supervise Mentors according to Fraser policy and procedures.
- Ability to achieve and maintain a status of “not disqualified” upon completion of criminal background studies.
- Possession of a valid Minnesota Driver’s License and maintenance of a satisfactory driving record consistent with Fraser policy.
Approximate Minimum Starting Wage: $30,000.00 Annually
