Supportive Living

Fraser Supportive Living provides an option for adults who want to live more independently in the community and require less support than a typical group home. Services are delivered to individuals in their own homes or apartments. Roommate opportunities may also be available. Supportive Living services are funded by the Developmental Disability (DD) waiver program. Rent and utilities are funded by tenant wages, Supplemental Security Income (SSI), and Minnesota Supplemental Aide (MSA) Housing Assistance.

Fraser staff provide:

  • Evaluation of the person's needs and skills
  • Roommate matching assistance
  • Daily or periodic direct care services, depending on supervision needs
  • Person-centered planning
  • Independent living assistance (budgeting, cooking, safety, medication administration, etc)
  • Community integration assistance
  • Overnight Staff
  • Monitoring technology
  • On-call emergency assistance

A successful candidate for Fraser Supportive Living will be comfortable with some time alone, be able to react appropriately in cases of emergency like tornado or fire (training provided), and will not be currently exhibiting patterns of destructive or aggressive behavior.
