Eligibility

Adults who have developmental disability waiver funding are eligible. Additional eligibility criteria may be required depending on the site.

Our Staff

The program is supervised by a coordinator with a bachelor’s degree and minimum two years experience in developmental disability services. The coordinator supervises and trains direct support professionals to ensure program goals are met.

Payment/Funding

Payment options include Developmental Disability waiver funding and private pay.

Current Openings: Hopkins Plaza - Hopkins, MN

Hopkins Plaza is a three-bedroom townhouse for adults with developmental disabilities. There is an opening for 1 adult female. Recently opened, Hopkins Plaza is located near downtown Hopkins, allowing easy access to employment and amenities. Fraser staff will provide Supported Living Services (SLS) using DD waiver funding. The site will not be foster care licensed; therefore, no GRH funding is available for room and board. Rent is $420 per month and is paid directly to the landlord. Additional living expenses (~$100 per month) are also paid for privately by clients through SSI, wages, or other income. Current Openings: Greenfield Apartment - Hopkins, MN

Greenfield Apartment is a three-bedroom apartment for adults with developmental disabilities. There is an opening for 1 adult female. This beautiful apartment has wonderful amenities, including indoor and outdoor pools, tennis courts, a fitness center, business center, and nature preserve. Fraser staff will provide Supported Living services (SLS) using DD waiver funding. The site will not be foster care licensed; no GRH funding is available for room and board. Rent is $405 and living expenses (~$85 per month) are paid for privately by clients through SSI, wages, or other income.
