Transition Services

For youth who are on the autism spectrum, transitioning to adulthood can be challenging. Fraser Transition Services work with individuals and their families to help them better understand and address the key issues of education, employment, community living, and community integration.

This program serves teens and young adults 15-24 years old.

Transition Services can include discussions related to:
  • Nurturing independence
  • Forming and maintaining mature relationships with family, friends and co-workers
  • Developing the skills necessary to cope emotionally and financially
  • Exploring post-secondary education, training and life-long learning
  • Investigating residential options
  • Reviewing strengths and interests as the precursor to employment
  • Building routines that promote health, wellness and recreation
  • Navigating resources available through local agencies, schools and the internet


Locations: This program is currently offered in our Fraser Minneapolis, Fraser Anoka and Fraser Bloomington locations.

Payment: Payment is arranged on an individual basis (private pay, school, agency, etc.), and may be covered by bundled contracts through insurance or incorporated into skills training services.
